Mission

The goal of the CHR/Diabetes Program is to address health care needs through the provision of community-oriented primary care services, including traditional and cultural concepts in multiple community-based settings such as transport to any health care facility, home visits, schools and community centers by utilizing our community-based, well-trained, guided health care workers.
